Aging Wisely – How Legal Documents Secure Quality of Life Goals

Legal documentation can offer peace of mind for caregivers, but how do you know what you need to complete to meet your care goals?

In this free online workshop, we will demystify these legal documents, so you can make them work for you. You will gain an understanding of informed consents, Advance Health Care Directives, Durable Power of Attorney for financial and health care decision-making, Physician’s Orders for Life-Saving Treatment (POLST), and medical aid in dying protocols. We will provide practical advice for maintaining legal documents and strategies to coordinate care across professional support teams.

This workshop is part of the Aging Wisely class series, providing a compassionate environment to explore your questions and concerns and help you gain a holistic perspective on how to approach long-term care planning for yourself or a loved one. Through presentations and group discussions, you will learn strategies and tools to help you assess your specific long-term care needs and explore solutions to common questions related to care planning.

Whether you are a caregiver to others or interested in your own future care, you will have the opportunity to develop a customized long-term care roadmap supporting you to successfully plan for care, with the input, advice and support of an expert in gerontology.

This class will be recorded and a one-week link to the recording will be shared with all registered participants. Attend the live session or listen to the recording within one week to receive incentive points.

Instructor: Instructor: Paula Wolfson is a licensed clinical social worker and is the manager of the Care Partners program at Avenidas in Palo Alto. She is an expert in navigating health care transitions, caregiver support, and legal, financial and health care resources. Paula has supported the Stanford community for many years, offering individual consultation and leading workshops through the WorkLife office.
