Aging Wisely – Geriatric Health Care Providers and Community-Based Elder Care Services

This class is co-sponsored by the Stanford WorkLife Office.

As we age, our health needs shift and can change rapidly. How can we set up a comprehensive yet flexible plan for care?

In this free webinar, we will help you build your knowledgebase of geriatric health care and elder services, allowing you to seek out the care that is needed for the current moment and be prepared when things change. We will review strategies for creating continuity of care between your skilled medical care providers and outpatient community elder care services, offer patient care advocacy tips for navigating discharge planning, and learn the importance of early detection of cognitive change.

This webinar is part of the Aging Wisely class series, providing a compassionate environment to explore your questions and concerns and help you gain a holistic perspective on how to approach long-term care planning for yourself or a loved one. Through presentations and online discussions, you will learn strategies and tools to help you assess your specific long-term care needs and explore solutions to common questions related to care planning.

Whether you are a caregiver to others or interested in your own future care, you will have the opportunity to develop a customized long-term care roadmap supporting you to successfully plan for care, with the input, advice and support of an expert in gerontology.

This class will be recorded and a one-week link to the recording will be shared with all registered participants. To receive incentive points, attend at least 80% of the live session or listen to the entire recording within one week.
