The 2012 BeWell Program

Start with SHALA

The Stanford Health and Lifestyle Assessment (SHALA), an online health risk assessment, is the gateway to the BeWell program. Take 20 minutes to complete the survey and you will instantly get a personalized report that outlines your healthy habits and ways you could be healthier in the future. Benefits-eligible employees who complete the SHALA are eligible to move on to the Wellness Profile and BeWell Berries.

This year, spouses and registered domestic partners of eligible employees are welcome to take the SHALA and participate in the Wellness Profile.

Wellness Profile

1. Screening:  Regular health screenings can reduce your risk for heart disease, cancer, diabetes, and metabolic syndrome. BeWell's trained staff provides free one-on-one health screenings on campus at a convenient date and time for you. If you prefer, you can have your screenings done at your doctor’s office (with a co-pay) and have your provider complete a form and return it to the BeWell office. The specific values measured are total cholesterol, HDL, glucose, body mass index (height and weight), waist circumference, and blood pressure.

2. Advising:  A wellness advisor can help you create short-term goals and direct you to available resources on campus. BeWell provides a free one-on-one advising session conveniently scheduled immediately after your screening appointment. If you elect to have your health screening information submitted from your doctor’s office, you may schedule your appointment after BeWell has received and recorded the results.

 3. Plan:  Having a wellness plan helps keep you on track. BeWell provides you with a personalized, online planning tool to identify your goals and strategies.

Benefits-eligible university employees will receive a $480 (taxable) wellness reward in 2013 for (a) completion of the SHALA and Wellness Profile, (b) agreeing to share information in 2012, and (c) enrollment in a Stanford-sponsored medical plan in 2013. Benefits-eligible employees receiving $480 in 2012 (for completing the SHALA and Wellness Profile in 2011) who do not agree to share information or are not enrolled in a Stanford-sponsored medical plan in 2013 will not receive the $480 additional financial incentive in 2013. Benefits-eligible employees who did not complete the SHALA and Wellness Profile in 2011 will receive $200 for completing them both in 2012.

Spouses/registered domestic partners of benefits-eligible university employees will receive a $240 (taxable) wellness reward in 2013 for (a) completion of the SHALA and Wellness Profile, (b) agreeing to share information in 2012, and (c) enrollment in a Stanford-sponsored medical plan in 2013. Spouses/registered domestic partners of benefits-eligible university employees that do not agree to share or are not enrolled in a Stanford-sponsored medical plan will not receive a financial incentive in 2013.
